Returns all billable services identified in the documentation, including:

• E/M codes — office visits, hospital visits, consults
• Procedure codes — specialty-specific CPT and HCPCS codes (surgeries, imaging, injections, etc.)
• Diagnostic codes — ICD-10-CM codes linked to each procedure, supporting medical necessity
• Modifiers — applicable CPT modifiers (e.g., modifier 25 for significant, separately identifiable E/M)

Each charge includes a clinical reasoning statement explaining why the procedure is
supported by the documentation.
